Our practice receives payment through a monthly subscription fee. We offer a variety of packages to choose from that may fit your needs best. Please click on the rates page under the office policies menu above to see what we have to offer. Our main plans below include limitless primary care services personalized for your medical needs:

  • Monthly Membership Retainer 100/month
  • Up Front Yearly Discount 960/year
  • House Call Upgrade 175/month
  • Up Front Yearly Discount House Call 1680/year
It is possible for the fees to be considered a Qualified Medical Expense that can be applied to your Health Savings Account (HSA), Flexible Spending Account (FSA), or applied toward your insurance plan deductible. Please check with your HSA, FSA or insurance plan to confirm.

*We are not an insurance company, however New York regulations mandate that we charge a per visit copay of $30. This includes in office and house call visits. This payment is waived annually for the yearly physical. There are no copayments for televisits.
